• Vital Statistics Worksheet

This worksheet has questions that help us complete the death certificate for you. Please feel free to print this form and complete the information and bring to your appointment.


• Newspaper Obituary Worksheet

This should be the first priority for the family to work on. If you intend to have an obituary, please download and print this worksheet. It will help you with names and information needed for us to compose the obituary with you.


• Disposition Affidavit

If you are Out of the area or are unable to come in, you may:

• download, print and sign this form

• Then please call us so we can arrange to bring your loved one to our mortuary.


• Request Sheet

This form will give you information as to what you may need to bring to your appointment at Duggan's Serra.


• Authorization for Disposition With or Without Embalming

This form needs to be signed by the next of kin for preparation and embalming for visitation or family viewing. Time is of the essence, preparation should be done as soon as possible.
